Why facial anatomy matters
Every feature you recognize in a face — the height of the cheek, the curve of the jaw, the fullness of the lips — is the visible expression of structures hidden beneath the skin. When those structures shift, deflate or stretch, the face changes. That is why an experienced surgeon never thinks about the face as a single surface to be tightened, but as a set of layers that must be understood individually and treated as a whole.
Anatomy is also a matter of safety. The face is densely packed with nerves and blood vessels in predictable but delicate locations. Knowing exactly where they run is what allows our board-certified surgeons to work with confidence and precision.
The five layers of the face
From the surface inward, the face is organized into five well-defined layers. Surgeons learn them as a fixed sequence because the order never changes, no matter where on the face you are:
- Skin — the outer envelope, made of the epidermis and dermis, responsible for texture, tone and elasticity.
- Subcutaneous fat — the superficial fat layer that softens contours and gives the face its youthful roundness.
- The SMAS — the superficial musculoaponeurotic system, the fibrous layer that carries the muscles of expression.
- Retaining ligaments and spaces — the anchors and glide planes that hold the soft tissues in position over the bone.
- Deep fat and bone — the deep fat pads and the facial skeleton that provide the underlying framework and projection.
These same five layers repeat across the scalp, forehead, cheeks and neck, which is why surgeons can move confidently from one region of the face to another.
The SMAS and the muscles of expression
The SMAS is arguably the most important layer for facial surgery. It is a continuous sheet of fibrous tissue that wraps the muscles of facial expression and links them upward to the skin and downward to the deeper structures. When you smile, frown or raise your brows, the SMAS transmits that movement to the surface.
Because it is both strong and continuous, the SMAS is the layer that modern face lift techniques reposition. Lifting and supporting the SMAS — rather than simply pulling the skin — is what produces a result that looks rested and natural rather than tight.
The facial nerve
Powering all of those muscles is the facial nerve, which exits the skull near the ear and fans out into branches that travel within and beneath the SMAS. These branches are fine and must be protected meticulously during any facial procedure — one of the clearest reasons facial surgery should be performed only by highly trained, experienced surgeons.

Fat compartments and retaining ligaments
For a long time facial fat was thought of as a single, uniform cushion. We now know the face contains distinct fat compartments — separate pockets of superficial and deep fat, each bounded by thin walls. Some sit high in the cheek, others around the mouth, the temple and the jaw. They do not all age at the same rate, which is why the face can look hollow in one area and heavy in another at the same time.
Holding everything in place are the retaining ligaments — short, dense fibers that tether the soft tissues to the facial bones. The key ones for a surgeon are the zygomatic, masseteric and mandibular ligaments. In youth they keep the cheek fat high and the jawline crisp. As they lengthen with time, the soft tissue is free to slide downward.
How the face ages
Facial aging is far more than loose skin. It is a coordinated change across every layer at once:
- Bone — the facial skeleton gradually resorbs, reducing support around the eyes, midface and jaw.
- Deep fat — deep compartments deflate, flattening the cheek and creating hollows.
- Superficial fat — the surface fat descends, pooling along the jaw to form jowls.
- Ligaments — they stretch, allowing the midface to drop and the nasolabial folds to deepen.
- Skin — collagen and elastin decline, so the skin thins, creases and loses its snap.
Seen this way, it becomes obvious why simply pulling the skin tighter rarely looks natural. Effective rejuvenation restores volume where it has been lost, repositions tissue that has descended, and refreshes the skin — addressing several layers rather than one.
Anatomy and the natural result
